I have.  Usually 50 dollars/day is the max to "stay under the radar".  At the time I was doing quite well on the slots and made several hundred at different online casinos (at least I thought they were, actually they were part of the same company) and got banned from them.

In short, keep winning and withdraw money every two days, this way it won't hurt you much if they ban you.

Try to lose on purpose some bets, put bets on red or black, try to not play for a few weeks... these are some tactics that would help your account stays alive

Publicly traded casinos such as William Hill Group. Do not ban you.  They limit your play by adjusting the limits on your account.  On the sport book side they limit the wagered bets up to the opposite bet to equal out the vig, on roulette they limit your max bet options.  I had a max bet option of up to $5000 per inside bet with a maximum bet of $150000.  My limit is now down to a maximum $800 bet with a $25000 total bet.  They never refused to pay any amount, even a $60k football parlay.  Had it transferred into my account the next day.

Same for me with WH. last 14 months I had a payout of 800-1k every month. Still 12k on my account and never faced any problems there.
