I need some help with my Excel file. I cannot figure out why I am getting the N/A error in part of my spreadsheet.

Then I need help finishing the rest of my spreadsheet with the columns on the far right. Any help someone can give me would be GREATLY appreciated. Thanks for your help.

Your lookup formula is wrong.
